  • Please advise which branch you intend to deliver the repair to.
  • Please mark items that will be included for service. If the item is not related to your issue - please hold onto it.
  • Include any Items not listed above.
  • Please enter the serial number (not IMEI) of the device. Accessory items require unit serial or Proof of Purchase for coverage.
  • Describe in as much detail as necessary for us to understand the problem your product has, or the service required. If the problem is intermittent, what conditions does it occur under? Battery Power, Only running certain applications, etc?
  • Do you have your data backed up? If you do not, and do not request data to be retained, it may be wiped during service work.
    NOTE: Service Plus takes no responsibility for loss of data under any circumstances.
  • IMPORTANT: Service Plus takes your privacy and security seriously. As diagnostics may require access to your device, you should remove the password/passcode, and any sensitive data. If you provide any credentials, you agree that you will change them once you receive your device back from servicing.
  • Standard assessment fees for non warranty and out of warranty assessments (incl GST)

    Mobile Devices:
        - Apple iPhone/iPad/iPod - $50
        - Apple Watch - $50
        - Beats Audio - $50
    Computers:
        - Laptop / Desktop - $90
        - Apple / iMac - $90
        - Philips Monitors - $50
    Urgent Assessment - Jump the queue for initial assessment within 2 business hours.
        - Mobile Devices - $50 additional fee
        - Computers - $110 additional fee
  • Will you deliver/courier the unit to us, or would you like to utilise our ePak or PlusPak courier service?
            ePak - Up to Laptop Size - $15 within Island
            PlusPak - Up to Laptop Size - $25 within Island
            Desktop/iMac/Monitors - please call to enquire

  • Apple Devices must have Find My Device turned off for servicing.
    A non-refundable assessment fee applies for all non-warranty and out of warranty cases.
